Before signing a Service Agreement, please make sure you have read, understood and agree to our Terms of Service.
These form part of the agreement with EverAbility Group. Failure to meet your responsibilities as listed in the Terms of Service may result in withdrawal of services.
Signing a Service Agreement
When you choose EverAbility Group as your service provider, we will create a Service Agreement together. This is a written agreement between a service provider (us) and a customer (you), or their authorised representative.
Our Service Agreement outlines the supports we will provide and how they will be delivered. It also details the agreed budget or cost for providing services and supports.
A Service Agreement must be completed and signed before we can start delivering services.

Future changes to the Terms of Service will be communicated electronically via text message (SMS) and email. We’re moving towards paperless contact where possible. If you have changed your email or mobile phone number recently, or would prefer to receive these notifications in another format (e.g. Braille or letter) please use the form below to tell us about these changes.
